Dental-vision plan offers individual, two-person and family rates

Indiana Farm Bureau Health Plans offer a combined dental and vision plan designed to meet members’ needs no matter their age.

The plan is offered through the Delta Dental PPO Plus Premier network.

Dental benefits through another provider are built in to one of INFB Health Plans’ Traditional Health Plans, but the combined dental-vision plan can be added to other plans or used on a standalone basis.

The monthly rates are:

  • Individual subscriber – $49.75.
  • Subscriber plus additional person – $87.50.
  • Family (three or more people) – $146.

At the individual rate, that works out to less than $1.65 per person per day, noted Patrick Williams, INFB Health Plans manager. He added that the family rate is the same no matter the number of children being covered. “So for larger families, this is a huge savings compared to individual rates.”

The standalone dental plan has graduated coverage. The total maximum benefit for the first year is $500 per person, for the second year it’s $1,000 per person and for the third year it’s $1,500 per person.

“The vision plan has a very large network of optometrists and frames providers,” Williams added.

Both the vision and dental plans have waiting periods, but these don’t apply for semi-annual dental cleanings and annual vision exams.
